The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service has a vacancy for a dynamic, energetic and forward thinking Band 4 Children, Young People and Families Practitioner working 30 hours per week based at Shelton Primary Care Centre.


The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service offers an integrated, flexible and family focused service providing high quality, compassionate personalised support and care to Children and Young People across Stoke-on-Trent.

You will be part of a team delivering the 0-19 Healthy Child Programme for a varied, unique and diverse population, in supportive skill mix teams, working closely with our partner agencies across health, social care and the local authority.

The 0-19 Children's, Young People and Families Practitioner will work within uni/multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs.

They will contribute to the delivery of the Healthy Child Programme and to the improvement of the health and wellbeing of the population served.

The nature of the service will be determined by local service specifications.

The 0-19 Children, Young Peoples and Families Practitioner will work in a variety of settings including the Hub providing advice, information, guidance, signposting and referrals.
