Description

Purpose:


  • To build relationships with healthcare staff to identify unpaid carers in hospital settings, whether they be a patient or more likely the carer of a patient. Then to provide them with the support required to manage caring following hospital discharge.

Specific Responsibilities

  • Work in partnership with ward staff to identify carers of vulnerable patients who will have to be cared for post discharge
  • Provide service information and support to carers on wards
  • Post discharge complete person centred assessments and develop individual support plans, identifying specific support needs, gathering initial and final outcome scores
  • Signpost and refer carers to relevant support services ensuring that there are no unmet needs
  • Refer carers, where appropriate, for a statutory Carers Assessment in line with the Care Act 2014 guidance
  • Upload carers information to the online central management system (CMS)
  • Refer carers to Volunteer Hub to offer weekly welfare calls for a period of up to six weeks following on from their initial referral
  • Promote the service to professionals, delivering awareness briefings as required, or as identified by the Service Manager
  • Attend Multidisciplinary Team, Discharge Hub and System Flow Meetings on wards
  • Support the Service Manager to deliver accurate management reports, demonstrating the impact and performance of the project
  • Gather and write stories and testimonies of carers and patients who were identified and supported
  • Achieve individual performance management indicators as set by the Service Manager
  • Responsibility for promoting and safeguarding the welfare of children and adults
  • Provide support throughout the wider carers service as required
  • Support the induction, training and supervision of social work students and volunteers

  • Experience of sensitively discussing and gathering information from people who want to access support
  • Experience of supporting people to access information and services to meet their needs
  • Experience of providing individualised 11 support
  • Experience of working in partnership with peers to achieve an overall goal
  • Experience of using online recording systems
  • Experience of producing written reports to high and detailed standards
  • Experience of using Microsoft Office e.g. word, excel, outlook
  • Experience of working in the voluntary sector
  • Experience of raising awareness of a service to a range of audiences to increase referrals into services
  • Experience of supporting carers
  • Experience of supervising students/volunteers
  • Knowledge and understanding of carers rights
  • Knowledge and understanding of carers assessment process

Education /Qualifications

  • Good standard of English and mathematical skills
  • Qualification/ training related to the support of adults and/or young people
  • Safeguarding adults L1 & L
  • Safeguarding children L1

Other Circumstances

  • To have use of a private vehicle and be able to work across Rochdale
  • Flexible and able to undertake duties outside of the hours Monday
  • Friday 9am 5pm.
Successful applicants will be required to undergo a Basic DBS check.
