You must have great communication skills as you'll be the main point of contact relating to the discharge of patients on our ward areas, working with other members of the DISCO team, ward multi-disciplinary team (MDT) and external partners.

You must have knowledge of no Criteria to reside and pathways to support safe discharge into the community

The Discharge Facilitators play a vital role to support our ward teams to achieve timely discharge of our patients.


You will ensure discharge planning commences from day 1 of admission liaising with the multi-disciplinary teams and family to ensure patients spend the minimum amount of time required in hospital.


The post holder will be expected to undertake a range of duties without direct supervision but will be required to report back on delegated duties to the ward areas and The Discharge Specialist and trust Lead for hospital Discharges where required.

you will need to be competent to work across multi-professional disciplines, making referral for additional assessments and treatments where required.


You will need to use your initiative most of the time but you will also have a supportive team around you to help.

It is important that you use your organisational skills to manage your workload and ensure tasks are completed timely and escalate concerns.
