We are seeking a highly motivated individual to come and join us in the Plymouth Diabetes Specialist Team, working across the community covering Plymouth and South Hams.

We are a diabetes and Endocrinology unit based at University Hospital Plymouth NHS Trust.

Main duties of the job

Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
Assist the Diabetes Specialist Nurses in the assessment, education and training of patients and their formal and informal carers.

To assist with teaching of diabetes self-management skills and provide health education to patients and their family/carers.


To use IT systems to support the early identification of inpatients with diabetes, assist delivery of care, planned, and assessed by the Diabetes Specialist Nurses.

To carry out care/other duties under indirect supervision of the registered nurse

To supervise health care assistants, contributing to their training and development


Job Purpose:

Key Dimensions:

Support care on a day-to-day basis for all patients in the clinical area under indirect supervision.

Patients may be seen as an inpatient and in the outpatient and other clinical settings.

Contribute to resource management

Completion of diabetes competency pack to level appropriate to this post
