Description

The post holder carries responsibility for the assessment of care needs and their development, implementation and evaluation of programmes of care for clients with mental health problems who are referred to the Crisis Resolution Home Treatment Team (CRHTT).

This post is primarily for East CRHTT, based at Rochford Hospital, however the post holder may be required to work across the service and be placed in the West CRHTT, based in Basildon, should a necessity arise.


The post holder is responsible on a day-to-day basis to the Team Lead but ultimately accountable to the Team Manager for the overall service.

In addition, they will be a member of the multi-disciplinary team and work in close co-operation with wards and community teams.

They will be responsible for the maintenance of standards of care within CRHTT; sharing responsibility for an integrated service provision to the clients and ensuring that a good inter-disciplinary working environment is maintained.
