MAIN DUTIES AND RESPONSIBILITIES: 1. To demonstrate the Trusts values in everything you do in the work environment and live up to the LPT Pledge. 2. To be responsible in the use and expenditure of the Trusts resources that you utilise.


  • Delivery of Effective Healthcare within the organisation 3.1. To provide clinical direction and leadership to staff within the specialist continence team. 3.2.
To support the service in the assessment, development, implementation and evaluation of holistic programmes of patient care. 3.3.

To ensure the team promotes and contributes to effective multi-agency assessment, for example the multidisciplinary approach that will deliver an end to end continence service.

3.4.

To work alongside the clinical educational team and contribute and support the specialist training in care homes. 3.5.

To work closely with the Community Services Manager/Community Services Matron to promote and facilitate developments within the specialist continence service to meet the health needs of the local population and commissioning intentions.

3.6.


To ensure the teams caseload is regularly reviewed and the skill mix of staff is able to deliver the integrated flexible working patterns that support clinical pathways and future service models.

3.7. Balance operational management and clinical role to deliver an effective service. 3.8.

Lead where appropriate the developments of clinical guidelines, protocols and care pathways. 4.2.

Lead the development and implementation of an effective specialist practitionerled service across LLR and in line with current best practice.

4.3.

As required provide senior clinical advice relating to patients with complex needs to multi-disciplinary teams and Community Hospitals. 4.4.

Make informed decisions regarding the progression of the Continence specialist service and anticipate and plan for effective and efficient utilisation of resources within the service.

4.5.


Demonstrates on a daily basis a high level of intuitive and creative problem solving skills to facilitate a high standard of patient-centred care.

4.6.

Adapts to the demands of Organisational change of own and other professional bodies to meet deadlines and achieve positive outcomes.

5.

Continuing Education, Professional and Personal Development 5.1.

Undertake the Trusts corporate and local induction, and maintain your learning and compliance with training requirements for your role 5.2.

Participate in supervision via agreed review and appraisal mechanisms 5.3.

It is mandatory for all professionally qualified staff and clinical support staff to actively participate in clinical supervision as an integral part of their professional development.

5.4. Be responsible for the recognition of learning needs and contributing to learning outcomes of all levels of staff. 5.5. Liaise with all colleagues in a teaching capacity to ensure an overall coordinated approach for team and staff development.

5.6. Develop his/her managerial skills in accordance with an agreed development plan. 6. Clinical Governance, Reduction of Risk, Audit and Research 6.1.


To support the Community Service Manager/Community Service Matron in the investigation of complaints/concerns or Serious Investigations (SIs) in accordance with LPT policy.

6.2.

To take responsibility for recruitment, induction and retention of staff ensuring regular review of skill mix, service capacity and clinical expertise.

6.3.

Completes risk assessments 6.4.

Responsible for and maintains health, safety and security of self and team in own work area for example, lone working, control of substances, manual handling, infection control, risk management, fire and vulnerable people, RIDDOR.

6.5.

To support the implementation and monitoring of effective induction and preceptorship programmes, ensuring an effective programme of appraisal and staff development.

6.6.

Manages self and team to submit clear, accurate contemporaneous records both manual and electronic in accordance with Information Governance procedures and Trust Policy.

6.7.

To manage infection control issues within the scope of Continence and oversee relevant audits, writing action plans to meet any perceived variances across the service.


  • Management and use of Resources and Information 7.1. All records that the role is responsible for or modifies must be kept up to date and maintained in an accurate and diligent manner. 7.2.
Monitor and review current expenditure of containment products and continence equipment used within the Organisation. 8. Operating with Quality in everything you do and Maintaining a Safe Environment 8.1. Challenge existing knowledge, current poor practice and be open to be challenged by others.

8.2.

Use equipment required for any procedure detailed in clinical guidelines and policy or that which is required within the speciality 8.3.
