What you'll be responsible for:


  • To upload triaged referrals to Buckinghamshire Council portal, LAS, for service finding or Social Worker allocation. To record all actions on the appropriate live tracker. To move referrals to appropriate, paper free, folder.
  • To be the first point of contact for referrers contacting CCCT by telephone, for patients who require input from the community services community hospital bed, Adult Community healthcare Team, Bucks Care Reablement for nursing interventions, reablement, intermediate care and rehabilitation, Community Hospitals and other hospitaltohome initiatives; to answer straightforward questions, and to pass these calls on to the appropriate clinician where necessary.
  • To undertake telephony duties in relation to the management/ triage of patients referred to the service.
  • Duties as directed by the CCCT Clinical Lead, planning and prioritising own workload.

You'll learn the following whilst working at the trust:

  • An in depth understanding of the roles and responsibilities involved in working within the NHS
  • Knowledge of the systems used, to effectively complete your role to the highest standard at all times
  • A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation

You'll have the following skills/experience:


  • IT literate and proficient with word processing, database and spreadsheet packages including Microsoft Office Word, PowerPoint, Excel, and MS teams.
  • Excellent time management and organisational skills
  • Excellent written and verbal communication skills
  • Significant experience in a busy administrative environment
