Job Introduction

A golden job opportunity is available as a Lead Support Worker within the JCR Reablement team in Bexhill, Hastings and Rother.

This is an exciting development opportunity for someone who is organised, caring and has a good eye for detail.

Please see attached job description but as a Key Worker to the clients, your main responsibilities will include:

Complete the first visit, explain JCR service provision, agree goals with the client and plan how staff will support the client to achieve these.
Weekly visits to review and monitor client progress
Discussions with the clients to establish and review their personal goals
Recognising barriers to progress and taking the necessary steps to overcome these
Working closely with health and social care professionals, including therapists within the JCR team.
Support clients with specific activities for example gait education and exercises
Liaising with family / Next of Kin on the client's behalf with the client's consent
Reporting to Duty team to ensure timely updates
Working closely with Team Leader and other professionals where complex needs are identified
We can offer you full training to learn the role and we support your development with a dedicated mentor. You will be part of a friendly team and you will shadow others in the role first. You will work closely with a Team Leader as you manage your group of clients together and monitor their progress.

This role is great way to progress in your career from a care role and take on some more responsibility as well as making key decisions in the client's recovery.

Your starting salary will be pro rata if the above position is part-time or term-time only.
