Job summary


An exciting opportunity has arisen to work in a newly developed Same Day Access service for Weymouth and Portland Primary Care Network (PCN).


The service will aim to manage acute on the day demand, initially for a defined number of conditions, but with the aim being to expand the scope in due course.

Main duties of the job
The role involves providing a combination of telephone triage and face to face appointments. The clinicians providing the PCN Same Day Access Service will work together from the same site. Support will be available from a duty GP if needed.

This role is ideally suited to Paramedics, Advanced Nurse Practitioners and Paramedic Advanced Practitioners.

There is scope for development within the role, particularly including non medical prescribing.

Job responsibilities
To be responsible for completion of the allocated triage calls and deciding on the most appropriate action for that patient

Assessing patients face to face as appropriate

Develop an understanding of what conditions require same day medical assessment and those that can be managed remotely

To be autonomous in clinical decision making

To have an awareness of when and how to seek senior review

To keep clear contemporaneous records

To formulate a management plan in partnership with the patient

To support with the ongoing development and expansion of the Same Day Access Service


Person Specification

Qualifications

Essential

  • HCPC registered Paramedic or NMC registered Nurse Practitioner.

Experience

Essential

  • Confidence to safely triage autonomously, without the support of a triage tool.
  • Experience in dealing with a wide range of conditions that may present daily

Personal Qualities

Essential

  • Motivated, hardworking with good communication skills and able to work well as a part of a team.

£40,588.00-£54,619.00 per year
