Lyndon 4 have an exciting opportunity for a Band 3 Patient Flow Co-Ordinator.


The role involves supporting a safe and effective patient journey and ensuring patients are discharged with the appropriate support in place or to a suitable accommodation.


It is a full time vacancy and the successful applicant will be required to work Monday to Friday, 7am-3pm or 8am - 4pm.


You will be working closely with multiple teams, including the trusts Integrated Discharger Hub, Palliative Care Team, District Nurses, West Midlands Ambulance Service etc.

so you will need to be a good communicator.

You will also be expected to regularly update patients and their families regarding ongoing discharge plans.
