Description

An opportunity to work within a small team of Physios, Nurses and Assistants delivering a comprehensive Cardiac Rehabilitation programme to people with a variety of cardiac conditions.

You will be responsible for supporting patients through their rehab journey. This will include mobility and functional capacity assessments, monitoring and documenting. You will also help to educate patients and encourage them to develop healthy lifestyle habits.


Furthermore, you will participate in service audits and you will have the opportunity to actively assist in service development projects.

Maintaining and assisting the development of the exercise component of the service according to national guidelines and research.

Leading group and one to one exercise sessions.

Monitoring and evaluating patient progress, adjusting the programme to gain optimum benefit for the patient.

Advising / teaching patients and carers on returning safely to preferred activities.

Taking responsibility for health & safety within the cardiac rehab environment.

Liaising with other health professionals both in and out of the Trust.

Contributing towards the development of the service and supporting the service leads and colleagues.

To work within the cardiac rehab team to deliver a safe and effective cardiac rehab programme for a people with a variety of cardiac conditions.

DETAILED DUTIES & RESPONSIBILITIES
Communication


To effectively communicate complex information in a simple way to ensure patients and carers understand their condition and how to maximise their rehabilitation outcome.

This requires experience in using motivational techniques and positive, non-judgemental approach.

To establish and maintain rapport with patients and carers.

To overcome barriers and source solutions towards effective communication, eg. to patients with hearing or visual impairments, anxious individuals, etc.

To facilitate and be part of a well-coordinated MDT for patient centred care.

To maintain accurate, comprehensive, and up to date clinical documentation, in line with legal and service requirements.

To establish and maintain healthy working relationships within but not limited to the team.

To develop motivational strategies in order to support and inspire patients, carers and other staff members.

To deliver presentations and in-service training and contribute to professional development forums.

To promote the Cardiac Rehabilitation service and to ensure a consistent approach to patient care.
Analytical Tasks


To undertake comprehensive assessment of patients with a range of clinical conditions, including those with a complex presentation, using analytical skills.

To utilising a range of intervention skills and options to formulate a comprehensive, evidence-based individual management plan.


To be able to analyse and interpret clinical and social information in order to make appropriate recommendations for other professional intervention if required.

To integrate this information into patient management.


To evaluate the patients' understanding in order to gain valid informed consent and have the ability to work within a legal framework.

This includes patients who lack capacity to consent to treatment.
Planning and Organisational Skills

To organise and manage a large caseload, with clinical priorities and delegate responsibilities to other staff as appropriate.

To assist in the mentoring and training of support of staff and students.

To co-ordinate and produce reports as requested for the area of work.

Physical Skills

To be able to lead a group or a one-to-one exercise session. This includes cardiovascular, strengthening, flexibility and balance exercising.

To be able to move and handle patients and pieces of equipment in a safe and effective manner.


To maintain up to date training and knowledge of moving and handling, basic life support skills and immediate life support skills.

To adhere to service policy including competence to use equipment.

Responsibility for Patient Care


To be professional and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.

To ensure a high standard of clinical care and to assist staff to do similarly.

To be responsible for identifying patient goals and development/monitoring of treatment plans.
