Do you want to be part of an innovative new project working within Mental Health Rehabilitation?


An exciting opportunity has arisen for the development of a new service working across both Acute Adult Mental Health, and Rehabilitation & Recovery to support patient experience, early rehabilitative interventions, and timely transition through services as part of recovery.


TheAdult In-Reach Rehabilitation (AIRR) serviceis a new project which will be comprised of an MDT of occupational therapists, psychologists, nurses and physiotherapists who will use an in-reach model of care into Mental Health assessment and treatment wards.


This pro-active model of care will support timely assessment and interventions of rehabilitation and therapy requirements for patients at the point of admission and throughout their treatment pathway, to ensure the most appropriate and least restrictive environment is identified.

The team will collaborate closely with ward based staff to ensure that patients receive the rehabilitation support they need in order to develop/maintain their independence and skills whilst they are in hospital.

This is aimed to reduce the overall amount of time patients spend in hospital settings and reduce the likelihood of readmission.


Duties of the role include, but are not limited to:

  • Being part of the development and delivery of a new model of rehabilitation care within the service group
  • Monitoring the performance of the service in line with set objectives and targets
  • Providing specialist advise and professional leadership to staff regarding the management of patients presenting with rehabilitation needs
  • To participate in audit activities as required the by the Mental Health Service Delivery Unit Audit Committee. e.g. Audit of specific local Policies and Procedures.
  • To be responsible for the implementation of all relevant policies/procedures that are applicable within the role and ward areas.
