An exciting opportunity has arisen for an experienced practitioner to join the Transforming Care Service within ELFT, which is focusing on those people with learning disabilities and / or autism, whose complex needs are supported within the Transforming Care framework in Luton, within the BLMK ICS area.


This is a role for a dynamic and compassionate, registered practitioner who is committed to implementing and managing, system wide processes that prevent unnecessary admissions to inpatient mental health or secure settings, and support timely discharge and/or step-down to community settings.

Knowledge of local and national initiatives relating to the Transforming Care Programme, is a key requirement of this job. Additionally, knowledge and understanding of the Mental Capacity Act, the Care Act and the Autism Act are also required.


You should have experience in working in the field of health and social care and be able to demonstrate significant experience of positive partnership working.


You will work alongside partners such as: Local mental health inpatient areas, Local CMHTs and ALDTs, NHS East of England Provider Collaborative, People who have a Learning Disability and / or Autism, Carers, Families and Children's Services.
