Description

The post holder will work as a member of the safeguarding team to ensure a high quality safeguarding service is delivered to all staff and patients.

This will include providing advice, support and relevant information to a multitude of professionals, enabling the safeguarding team to assess the most appropriate plan of action for each individual case reviewed.

Main Duties Through liaison, research and dialogue identify and collate relevant health information to add to multi-disciplinary risk assessment discussions and Multi-Disciplinary Team Meetings (MDT's).

Under supervision of the Head of Safeguarding, provide timely and accurate case conference reports and health safeguarding reports as requested, as part of the Coventry safeguarding children's board (NSCB) protocol.

Attend key meetings as required, such as the practice Multi

  • Disciplinary Team Meetings (MDT's). Collate accurate minutes of team meetings that are attended. Accurately record minutes onto patient records and safeguarding information database. Provide support and guidance to all staff with support from the Head of Safeguarding.
Under the guidance of the Head of Safeguarding, proactively review case files for individual patients in relation to levels z
- of Coventry Thresholds and Pathways procedure. Ensure patient records are maintained by the team, complying and adhering to the practice record keeping systems (policy) Obtaining informed and written consent prior to any information being shared with professionals outside of the Surgery
  • Unless this meets the requirements within the safeguarding children's Respondto requests for health information in a timely and professional manor. Contribute to the collection of data for safeguarding metrics that accurately reflects the safeguarding team activity. Participate in audit activity as required within the safeguarding team.
Manage and maintain the safeguarding information database, to ensure cases are reviewed in line with agreed timescales.

Raise any safeguarding concerns directly to the Head of Safeguarding or the Safeguarding practice Contribute to the delivery of safeguarding information and training Personal Development Receive supervision from the Head of Safeguarding on a regular planned basis in recognition to the potential impact of working in safeguarding on self and others.

Seek support and help where necessary.

Attend monthly 1-1 meetings with the Head of Safeguarding to develop clear actions plans of individual work and progress within the team.

Participate in yearly appraisals.

Work with the Head of Safeguarding to develop an individualised education plan to enhance knowledge and progression within the safeguarding team.

Attend a variety of venues within the county to ensure updated knowledge of Northamptonshire safeguarding working procedures/ practice requirements. Attend countywide training sessions as required or as identified within the individualised educational plan.
