Job Purpose Have a clinical role, with responsibility for planning, managing and co-ordinating the care of people with highly complex needs and long term conditions within a defined caseload.

Use caseload management techniques to reduced unplanned hospital admissions caused by poor control of long term conditions. Develop advanced skills and competencies in prescribing, health assessment, medicines management, leadership and case management.

Work collaboratively across Primary, Secondary, Social Care and the Voluntary sector to establish a whole systems approach to managing Health and Social care for the designated patient group.

Take a lead role in clinical practice and strategic development for Nursing and Primary Health Care teams.

Dimensions Managing an active caseload of complex patients with a variety of long term conditions from the registered population and clinically working with the wider community nursing team.

Networking and liaising with GP Practices and a variety of professionals across Health and Social care boundaries, Statutory and Voluntary organisations, patient and carers.

Spending a minimum of 75% of time on clinical activity.

Working with the neighbourhood team to meet the demands/needs of the service.

Key Responsibilities Clinical Using expert knowledge, advanced nursing and autonomous decision making skills manage a caseload of patients in the defined client group.

Comprehensively assess, review and evaluate the needs of both patients and their carers to improve their physical and psychological wellbeing whilst reducing acute exacerbation of underlying conditions and need for hospitalisation.

Work in partnership with patients, carers, GPs, consultants, other health professionals and social care as appropriate to instigate diagnostic testing and therapeutic treatments, ascertain diagnosis, develop, initiate and implement proactive care plans.


To develop advanced specialist clinical skills to undertake interventions consistent with evidenced based practice in line with national and local policy.

Use knowledge of medicines and prescribing skills to influence the reduction of risk and complications associated with medication and polypharmacy.

To work closely with prescribing advisors and pharmacies as appropriate.

To be responsible for the promotion of best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies procedures and guidance of the organisation.


Practice in accordance with the procedures and guidelines for the protection of vulnerable adults and child protection Ensure that confidentially is protected at all times.

Any other duties that may be reasonably required to support the needs of the service. Professional Actively engage in clinical supervision. Maintain a personal development plan and professional portfolio.

Participate in the PDR review process to identify training and development needs. Remain updated and ensure that clinical practice is evidence based. To be flexible within the role, working at different bases when necessary.

Managerial and Leadership Establish clinical credibility to influence professional and organisational boundaries acting as a role model for clinical excellence.


Work collaboratively and in partnership with other practitioners to lead and developments in professional practice ensuring that care practices, delivery and service development are evidence based.

Take an active leading role in the development, implementation and evaluation of protocols and polices consistent with national Standards and research evidence to support the development of community nursing.

Initiate the interpretation of research findings, national and local policy, assessing the implications for profession in collaboration with service managers.

Encourage and support innovation, extensive networking and sharing expertise.

Provide professional support and advice to colleagues and senior managers within the organisation. Contribute to the workforce and local delivery planning processes. Advise on professional standards and contribute to the Clinical Governance of the organisation.

Demonstrate and promote advanced clinical skills to ensure patients and carers receive the best practice possible care and outcomes through clinically effective practice across the community nursing service.

Contribute to the wider development of practice by participating in research, audit local and national presentation, networks and publications.

Assist in the development, implementation and evaluation of education programmes for primary care workers to provide the necessary knowledge and skills for effective management of long terms conditions.

Educate and empower patients and carers to identify early signs of change of condition, provide them with the necessary knowledge and skills to gain independence and make informed choices to safely manage their condition.

Participate in the future training of health students and staff
