An exciting opportunity has arisen to join an integrated team who deliver care across St Helens locality.


The COPD Care Co-ordinator supports and facilitates the seamless integration of a range of services, clinicians and partners to support local patients, providing integrated care for adults, coordinated care for adults in the community who are at high risk of inappropriate or unnecessary hospital admission.

These patients will be reviewed in the Community COPD Hub to confirm a diagnosis of COPD.

This exciting role is pivotal to the effective delivery of care and is the key interface between patients and service users, carers, primary, secondary and community care, social care and voluntary organisations.
