An exciting opportunity has arisen to join the award winning community heart failure team.

We are a forward thinking, dynamic multi-disciplinary team that deliver care within a clinic and home setting. for heart failure patients across Swansea Bay Health Board.


The role will involve assisting the specialist heart failure nurse in the assessment, ongoing planning, management and evaluation of care.

It will involve working predominantly in a clinic setting, across all hospital sites/ primary care clinic sites. The role will also involve some home visiting.

Main duties will be to undertake patient observations, ECG's, phlebotomy, preparing clinic work load for specialist nurse. You will require to attend home visits and provide some education to our patients.


The role will also require you to be able to order and ensure all stock supplies at all bases are fully equipped.

The Health Care Support Worker has a degree of autonomy and undertakes a broader range of health and care in accordance with policy and procedures.

The post holder will work on their own initiative, undertaking delegated tasks with appropriate supervision in place from the clinical nurse specialist clinical nurse specialist.

The post holder contributes to assessment and assists in the development, implementation and evaluation of individualised care plans for patients with heart failure.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
