JOB TITLE:
Team Leader


LOCATION:

West Sussex The service location outlined in your job offer, although you may be required to work from other locations at the discretion of the company and with appropriate notice.


SALARY:
£28,000- 30,000


HOURS: 40 hours over a 7 day week as determined by the needs of the service and notified via the senior management team.

Duties

  • To provide leadership to the support staff within the service
  • To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements and relevant regulations, under the direction of the Registered Manager
  • To supervise Client services within the Service in accordance with agreed standards
  • To ensure support is provided through the service in accordance with the requirements of CQC's guidance for "Right Support, Right Care, Right culture" Client care plans and the service's "We Will" statements.
  • To maintain skills at a current level, and undertake such training and development as may, from timetotime, be required to enhance your practice and provide additional skills for your role.
  • Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
  • To supervise staff and ensure that all staff contribute to the efficient running of the service.
  • To create a service culture of enablement and assist staff in their personal development to embrace the company values and ethos.
  • Ensure Safeguarding concerns are raised and managed according to company policy and guidelines from the WSCC safeguarding board.
  • Be responsible for promoting and protecting the welfare of those individuals supported by the service.
  • To support staff in the delivery of their duties and ensure that staff wellbeing is promoted.
  • Attend and participate in Client reviews and conferences.
  • To deliver care as outlined in the Client's support plans, PBS plans and Financial plans.
  • To ensure that support services are individualised and provided for the specified duration outlined in individual agreements and contract standards.
  • To safely support the ordering, stock, administration, auditing and error reporting of medication management.
  • Ensuring medication is managed within the principles of the 6 R's the right Client, the right drug, the right time, the right dose, and the right route, right to refuse.
  • To provide regular supervision and performance reviews to support staff which are supportive and developmental.
  • To complete staff working rotas based on the hours of support required each week and as required by the senior Management team on a monthly basis.
  • To calculate staff working hours each week and ensure these reflect the hours of support required for each client.
  • To ensure that the support hours provided reflect each Clients individualised support plans.
  • To monitor and evaluate Clients aspirations to ensure that the service continues to enable clients to live their life to the full.
  • To optimise activities, outings and holidays to reflect Clients hobbies, interests, aims, goals and enable clients to achieve or exceed the outcomes outlined in their care plans.
  • To act as ambassadors for the service and ensure that Client safety is maintained through effective risk management.
Working with Others

  • Ensure that clients are at the heart of support provision and reflects their wishes, preferences and wellbeing.
  • Develop effective and supportive working relationships with other employees within the service.
  • Ensure all staff working in the service are supported in their development and their wellbeing is maintained.
  • Responsible for ensuring that staff understand and are skilled at delivering the support outlined in each Clients care plans.
  • Provide key worker meetings weekly and monthly.
