£27,344 with the opportunity to progress to a maximum of £30,151

Do you want to work with young people with additional needs and their families as they move into adulthood? We want you to work in a dynamic creative service, delivering support to the citizens of Hertfordshire.

0-25 Together Service is a specialist servicefor disabled children, young people and their families


Our focus is to ensure we support people to make positive decisions in their lives, enabling their independence so that they can take part in their local community.

Joining us as a Preparing for Adulthood Practitioner, you'll be working with young peopleand their families and carers from the age of 14 to 25.

No day is the same. You will manage a caseload and work with a range of professionals involved in a young person's life.

Therefore, you'll need to be able to work under pressure and be extremely well organised, taking a pro-active approach to your workload.

No previous experience is necessary, but you will need knowledge of social care. You will need to be a car driver and able to travel. You will have to be able to work alone and also be an excellent wider team. You will also need to have good communicationskills.

This is a great opportunity to make a difference to a young person's life. It is busy and varied but an opportunity to help our young people move into adulthood.
