There is an exciting opportunity for an enthusiastic, proactive, organised individual to join our innovative physiotherapy team as a Health Disability Activity Practitioner.

You will lead the implementation and delivery of the Health Disability Activity Partnership across the Health Board area, including delivering the Health Disability Activity Pathway training, to achieve the outcomes of promoting improved health and well-being of disabled people through increased levels of physical activity as detailed within the partnership's National and local delivery plans.

Work as an individual Health Disability Activity Practitioner as part

of the local DSW Team across the Health Board footprint to develop local delivery plans adapted to suit the needs of the local population.

Work collaboratively with the National Health Disability Activity Team and DSW to create a range of resources (including training session resources) for health professionals to support them in promoting physical activity and utilising the Health Disability Activity Pathway, and for disabled people accessing health services to increase awareness of local physical activity
(including sport) opportunities and how to access to them.


To create, maintain and enhance effective working relationships with health professional colleagues across the Health Board area, including Primary Care and Social Services, Local Authority, and third sector partners to maximise the Health Disability Activity Partnership's contribution to health improvements in the community, reducing health inequalities, through increased levels of physical activity amongst the disabled population.


To create, maintain and enhance relationships with relevant service managers and decision makers to ensure the Health Disability Activity Pathway is embedded in appropriate service delivery plans and Job Descriptions.
