Main Responsibilities:

Develop relationships across our clinical team Build relationships with external service providers, creating a directory of services for our patients to access Speak to patients about their health and wellbeing Provide a signposting function into services in the community Undertake patients health checks using the self-serve equipment Show patients our symptom checker and how to use this Provide patients with resources and information on managing their condition Show patients how to utilise our digital wellbeing apps Promote self-care and support.

To represent OnePrimaryCare in a knowledgeable and professional manner at all times.

To identify own training and development needs in conjunction with your line manager and participate in training opportunities as directed.

To partake in the OnePrimaryCare Personal Development Review system.

Provide ad hoc support to the reception and administration team as required Any other duties commensurate with the grade and level of responsibility of this post

Competencies:

Experience in patient facing roles Experience of social prescribing Patient Navigation Experience of providing mental health related support and advice.

Works productively in a high pressured environment Writes clearly, succinctly and correctly Adapts to the team and builds team spirit Embraces the OnePrimaryCare Values in day to day work Understands NHS process/systems Strong communication skills.
