The post holders will work closely with the multi disciplinary team in their locality. They will offer interventions for older adults in their own home.

Caseloads will consist of a range people with enduring mental health difficulties, dementia's as well as shorter term interventions with people with functional illnesses.

The team promotes a recovery approach.
