If your answer is yes, then come be our next Assistant Team Manager in our South Locality Hospital Discharge Service.


LIVE-WORK-CARE We are reshaping our Hospitals and Integrated Services to provide a Locality model of working for our customers needing support under Discharge to Assess models of care in their own homes and in care home settings, as well as continuing toprovide on-site staff at the acute and community hospitals serving residents of Wiltshire.

The Locality teams will work closely with their Reablement Teams, Ongoing Teams and Community Health Teams. The teams consist of Social Workers and Social Care Practitioners. The leadership team in each Locality is formed of a Team Manager, Assistant TeamManager and an Advanced Practitioner.


About the role The Assistant Team Manager will be pivotal in supporting the shaping and identity of the new Locality Team, working with staff through the changes needed to provide a high-performing service that is responsive to the needs of our customersand building on integrated health and social care working practices.


The role will include representing the service within multi-agency meetings and participating in the development of the service's policies and procedures.

As an Assistant Team Manager, you will provide supervision and support to social workers and socialcare practitioners and ensure a culture of learning and development.

You will have a key role in quality assurance of the work of the team to meet legislative, regulatory and policy requirements and you will set and monitor performance standards in the team.


You will maintain an overview of the team caseload and allocations and take responsibility for the best use of staff and financial resources in a service that has high demand and works to prescribed timescales.

About you You will be an experienced practitioner holding a relevant health or social care qualification with professional registration. You will either hold a management qualification or be able to demonstrate managerial skills and ability.


You will have detailed knowledge of social care legislation and substantial experience working in either social care or health settings.

You will have good written and verbal communication skills and be a confident decision-maker. You will use your leadershipskills to motivate and encourage people and develop effective working relationships with our multi-agency partners.
