Job Title:
Support Worker

1 x Full time post hours per week

2 x Part-time posts - pro-rata

Sleep-ins are in addition to the set hours

Registered Manager and Line Manager


The Role:

You will be working as part of a team to provide personalised care and support to our Service Users within their own homes or supported living accommodation.


You will bring experience of following individual care plans and risk assessments so that service users are safeguarded at all times.


You will be responsible in identifying and delivering activities that meet the needs of service users individually or in groups so that they reach their full potential and independence where possible.


You will work closely with individual service users as their Key Worker by supporting them to access the community, activities, and other services.


Principal Responsibilities:
The role requires sleep-ins within the service users' residential home on a rota basis. On-call required, the responsibility for this will be shared with senior management on a rota basis.

Caring for and supporting service users by following care plans and risk assessments so their physical, emotional, and mental health needs are met.


Keep full, accurate and up-to-date records including risk assessments, care and support plans for service users and daily record of events while on duty.


To recognise and provide support to service users to help manage their own emotions and behaviours and where possible, to encourage and help them to do things for themselves e.g., eating, socialising, and managing their finances.

Administer or support with medications.


Assist service users with advice and support to help them manage their finances as well as providing support around the maintenance of their homes.

Act as Key Worker using a person-centred approach complete monthly reviews of risk assessments, care, and support plans.


In line with their care plans, assist with risk assessments for events / access to the community for service users individually or in groups e.g., leisure facilities, libraries.

Support service users with activities taking part where necessary e.g., shopping, days out, swimming.

Build links within the community so service users can access facilities and services e.g., social, leisure, employment, and education.

Safeguard service users at all times taking necessary actions immediately and reporting any concerns or issues to management.

Work positively with managers, colleagues, and others e.g., social workers and health professionals to fully support service users.

Job Types:
Full-time, Part-time

Part-time hours: 20-40 per week


Salary:
£11.00-£12.00 per hour


Day range:

  • Every weekend
  • Monday to Friday

Ability to commute/relocate:

  • Derby: reliably commute or plan to relocate before starting work (required)

Education:


  • GCSE or equivalent (preferred)

Experience:

Support Worker: 1 year (preferred)


Language:


  • British Sign Language (preferred)

Licence/Certification:

  • Driving Licence (preferred)
