We are looking to expand our established team with this exciting opportunity for a forward thinking, Pharmacy Technician interested in developing their career in primary care, improving patient outcomes related to medicines and reducing prescribing variation ensuring patient safety.


You will be motivated, and passionate about patient care and will require excellent interpersonal skills to engage the stakeholders in the implementation of operational excellence relating to medicines use.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply


To work under the management of the Prescribing Support Pharmacist to ensure a safe and efficient service is maintained, delivering a range of medicines management activities including medicines reconciliation and medicines history taking.

To liaise with pharmacy staff in all sectors and other healthcare professionals to ensure a patient orientated medicines management service is provided, conducting type 1 & 2 medicines reviews for patients identified by the wider multidisciplinary team or from other medicines management

initiatives.

To work closely with the lead pharmacist to adopt a team approach to medicines management, acting as a key member of the multidisciplinary team and the wider pharmacy team, providing medicines management advice to healthcare professionals, patients and carers.

To work with the assigned Prescribing Support pharmacist to develop and undertake medicines management audits, to support discussion within the practice and agree subsequent actions.

This post is fixed term for 12 months due to meet the needs of the service
