Key 2 Care are an established and growing provider of quality adult home and community care services operating across the East and West Midlands.
We are currently seeking a professional and dedicated Field Care Supervisor to join our busy Grantham Branch.

To outline the main functions of the role:
As a Field Care Supervisor you will be the ambassadors for Key 2 Care, ensuring we deliver a dignified, loving service, that offers customers piece of mind, bridging the gap between office and front-line care delivery
Field Care Supervisors are responsible for the efficient running of their own specific, defined areas and teams and should vary their hours of work in order to effectively monitor and support carers and customers.
Field Care Supervisors should spend approximately 95% of their working week in the field with the remaining 5% being spent in the office in order to complete the necessary administrative duties.
- Carry out customer needs and risk assessments in the customer's home environment
- Produce customer support plans
- Complete reviews on customers, assessing if their circumstances have changed along with resolving issues/concerns
- Complete falls and moving & handling risk assessment as they occur
- Complete Financial assessments
- To complete customer's first visit, to allow you to gain insight into customers required support and associated risks.
- Update, maintain, and audit customer folders and records
- Update and maintain customer information on the CareFree/Birdie electronic care management system
- Complete customer contracts and update the CareFree/Birdie electronic home care system with customer and care package details
- Undertake care staff observations, spot checks, mentor new and existing care staff, contribute towards their training and shadowing period providing constructive feedback
- Ensure clear and accurate records and communication systems relating to care delivery are maintained and effectively used
- Conduct regular checks on customer files to ensure all paperwork is up to date and feedback to the manager
- Audit daily logs and MAR's on Birdie and report/address any concerns
- Deal with customer complaints and ensure these are resolved within set timescales.
- Undertake customer outcome reviews
- Work in partnership with care coordinator's, district nurses, community matrons and other health care professionals to develop and improve the service.
- Provide leadership and support to care staff and act as a champion for customers' safety and dignity
- Address any Medication concerns with the office team and customers
- Make regular quality care calls to customers and stakeholders
- Report any customer or carer queries/concerns back to the office
- Identify and recommend areas of improvement within the business and how to implement them.
- Conduct yourself professionally and portray Key 2 Care Limited (also trading as Derbyshire Care Services) in a positive manner whilst working in line with our Policies and Procedures
- Take responsibility of all tasks and follow through until completed
- Determine training needs for staff members and support staff accordingly.
- Act as a catalyst for change and improvement in performance and quality
- Ensuring that Health & Safety and Manual Handling processes comply with standards at national levels.
- Record, analyse and distribute statistical information as required
- Actively participate in and contribute towards team meetings
In order to achieve your objectives you will be required to work flexibly throughout a 7 day week, including evenings and weekends, as part of your contract.
