An exciting opportunity has arisen for a highly motivated individual to join the Adult Integrated Care Dietetic team as a Band 6 Specialist Dietitian.

The team work across community settings in the boroughs of Lambeth and Southwark including intermediate care settings, care homes, domiciliary care and in general practices.

You will also develop skills in leadership and supervision, through leading on student practice education for the community team and providing clinical supervision to band 5 dietitians.

As a member of the team you will provide dietetic care to adult patients who are frail and undernourished.

This includes domiciliary and free-living patients who are malnourished or at risk of malnutrition, those with pressure ulcers and patients requiring nutritional support due to illness, long term conditions or social factors.

We work closely with members of the health and social care teams, and the voluntary sector to ensure malnourished patients receive the appropriate care and support in the community.


The team visit patients at home, and we also visit care homes, nursing homes, assisted living accommodations and Intermediate Care Centres in Lambeth and Southwark.

We provide a number of outpatient community clinics for patients whom are malnourished, prescribed oral nutritional supplements or seek dietetic advice relating to weight reduction, diabetes or other diet related concerns.


You will be working closely with the South East London Prescribing Support Dietetic team and our acute colleagues across Lambeth and Southwark including Guy's and St Thomas' acute dietetic teams.


We are focused on improving integrated care and patient pathways, and you will have the opportunity to work with acute and community multi-disciplinary teams to support improved pathways of care including frailty and long term conditions.

Guy's and St Thomas' is among the UK's busiest and most successful NHS foundation trusts.

We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.

Guy's is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016.

As part of our commitment to provide care closer to home, in 2017 we also opened a cancer centre and a kidney treatment centre at Queen Mary's Hospital in Sidcup.

St Thomas' has one of the largest critical care units in the UK and one of the busiest emergency departments in London.

It is also home to Evelina London Children's Hospital.


The Nutrition and Dietetic (N&D) Department is the largest in the country made up of over 150 staff including dietitians, dietetics assistants, nutrition nurses, psychologists and administrative staff.

We have considerable expertise within the department with members representing the profession on a number of regional and national groups as well as contributing to both local and national guidance and research.

To work as a member of the Adult Integrated Care Dietetic Team in community settings across Lambeth and Southwark

To provide dietetic care to adult patients who are undernourished requiring support with their oral nutritional intake. This includes patients who are malnourished or at risk of malnutrition, those with pressure ulcers and patients requiring nutritional support due to illness, long term conditions or social factor

To communicate and work closely with members of the health and social care teams, general practice teams and the voluntary sector to ensure malnourished patients receive the appropriate care and support in the community


To visit patients whom are housebound at home, and visit Care Homes or Intermediate Care Centres in Lambeth and Southwark as required.

To collaborate with other teams providing care and support to patients in these settings including integrated care teams and care home nursing teams


To provide an outpatient community clinic for patients whom are malnourished, prescribed oral nutritional supplements or seek dietetic advice relating to weight reduction, diabetes or other diet related concerns.

To communicate effectively with the GP clinical team and improve referral pathways to the clinic
