Hours:
Full Time, (37.5 hours per week)

1. Background


The NHS Long Term Plan describes the prominent role Primary Care Networks (PCN) will play in delivering proactive, personalised and more integrated health and social care for their local populations.

Our South Southwark PCN is made up of all 18 GP
Practices in Camberwell, Peckham and Dulwich, approximately 155,000 patients, supported by their GP Federation, Improving Health (IHL).

We are seeking a PCN Support Officer to support programmes of work which improve the care of our population. This would include supporting

  • Delivery of Call and Recall programmes
  • Support the set up and ongoing delivery of services and initiatives
  • Data and reporting requirements


We are looking to recruit a highly motivated individual to work within our team and support the day-to-day support offered to our practices.


2. Job summary
The post holder will action the following tasks, working closely with the Federation/PCN team:

  • Support the set up and delivery of new and existing services and projects e.g. call and recall programmes, extended primary care services
  • Communicate effectively with general practice and other partners and organisations
  • Provide relevant data for management and reporting purposes e.g. regular dashboards, monitoring reports, presentations at meetings
  • Respond to adhoc queries from practices and highlight new requests to the team for scheduling.
  • Support back office /business as usual functions as required

3. Specific Responsibilities
The post holder will be specifically responsible for the following:

  • Deliver tasks related to population call and recall programmes undertaken by the
PCN/GP Federation

  • Support delivery of existing services which may include set up of appointment books, sending patient reminders, and liaising with practices
  • Build good relationships with general practice and communicate effectively with staff to support delivery of projects e.g. responding to queries, attending meetings, undertaking practice visits etc
  • Support data search requests, as and when required. These includes running monthly dashboards, extracting data for service monitoring/planning, adhoc practice requests, internal IHL requests etc
  • Analyse data and provide summary reports
  • Support business as usual tasks for services delivered by IHL
  • Deliver specific tasks related to communication with the wider PCN. This could include data/reporting, responses to practice queries, etc.
  • Support preparation of regular reporting, by collating data and feedback
  • Support preparation for the Neighbourhood meetings and other regular engagement meetings by providing data required for agenda items.
  • Respond to adhoc queries from practices and highlight new requests to the Team

  • In depth knowledge and practical experience of using the Microsoft Office suite including Word, Advanced Excel and PowerPoint

Knowledge and Skills

  • Knowledge of how to use EMIS WEB and how to create searches and reports
  • Understanding of Information Governance principles, patient consent

EDUCATIONAL

  • To participate in a planned induction program for new staff and ensure that an appropriate period of support is given by the line manager.
  • To have awareness of the IHL vision and objectives.
  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
