Community CNS Team -Team Leader 8A 34 Hours per week

Inner Northwest Division -Pembridge Specialist Palliative Care Service.


The Pembridge Specialist Palliative Care team practice holistic palliative care taking a multi-disciplinary approach to physical, emotional, social, and spiritual needs of their patients and their families.

The service comprises of a community team, day service, outpatients, and inpatient unit (The inpatient unit is currently suspended to admission awaiting the outcome of the NWL Palliative Care Review).

An exciting opportunity has arisen to join our community team as a Band 8a Team Leader.

We are looking to recruit a highly motivated, experienced palliative care nurse to lead our team of community Clinical Nurse Specialists.

You will be working with a team of very experienced nurses.


The post holder will be working with the leadership Team on the transformation and development of the Pembridge Palliative Care Services in line with the North West London Palliative Care and End of Life Review.

The role is based at the Pembridge Hospice, St Charles Centre for Health and Wellbeing.

The role will combine clinical, managerial, educational, communication and research skills in providing leadership and direction for the clinical nurse specialists based at the Pembridge Palliative Care Centre.


The post holder will be responsible for:

  • The leadership and management of the clinical nurse specialists in palliative care (under the strategic leadership of the senior nurse manager) in order to ensure the highest quality of palliative care services to the community and palliative patients in bedded areas within the Trust (e.g. nursing homes).
  • Providing specialist palliative care clinical advice and expertise to the CNS team, other members of the multidisciplinary team, patients and families
  • Deputising for the Senior Nurse Manager.
  • Providing senior clinical input to the clinical nurse specialist team, including regular case reviews and joint visits.
  • Leading on the development and maintenance of the "OOH" system to provide access to sevenday specialist palliative care advice
