We are looking for a driven and innovative Discharge tracker to join our friendly team, working within the new system-wide Integrated Discharge Hub at Sandwell and West Birmingham NHS Trust.


We have an exciting opportunity for you to join the award winning Integrated Discharge Hub service, which is operational from 8am to 8pm, 7 days per week.


You will be an integral part of coordinating, planning and progressing daily hospital discharges, supported by the Integrated hub team.

The role will be focused around providing essential discharge support to the Acute medical wards, communicating clear messages between the hub and multidisciplinary team.

The role will significantly contribute towards quality assurance for hospital discharges, including acute and community care.

To support and work with the Clinical lead, Complex discharge nurse
specialists, Wards, Multi-Disciplinary Health and Social Care professionals,
and the Bed Management Team identifying, screening and tracking patients through the hospital systems.


To ensure that all National, Regional, Trust and Departmental policies and procedures are maintained within the remit of Delayed Discharges/Reimbursement protocols.

Ensure that any problems which may impede the speed, safety and effective
discharge of a patient are identified as early as possible and advised to the
individual Complex Discharge nurse specialist and/or Clinical lead as part of
an agreed escalation policy.

Person specification

Experience

Essential criteria

Experience in adminsitrative roles

Experience and good working knowledge of Microsoft systems such as Word and Excel

Desirable criteria

Previous NHS exerperience of supporting discharge processes
