Job summary
Primary Care Networks provide new and additional health and wellbeing services to our patients.

Working in partnership with our GP Practices, we deliver extra services such as wound care, physiotherapy, mental health support, wellbeing clinics, respiratory reviews, BP monitoring, frailty home visits, medication reviews and other innovations.

Middleton & Hunslet PCN was established in 2019, has a list size of approximately 33,000 and covers the following 4 GP Practices located in LS10:
Lingwell Croft Surgery

Southbank Surgery

Arthington Medical Practice

Church Street Surgery

The PCN Care Coordinator Phlebotomist hybrid role is seen as a critical and evolving post to support the Primary Care Network member GP practices and PCN Multi-Disciplinary Teams (MDTs) to deliver effective, co-ordinated care for patients.

This will involve a mix of working from GP Practices, community settings and home visits.

Job responsibilities

JOB PURPOSE


The PCN Care Coordinator Phlebotomist hybrid role is seen as a critical and evolving post to support the Primary Care Network member GP practices and PCN Multi-Disciplinary Teams (MDTs) to deliver effective, co-ordinated care for patients.

This will involve a mix of working from GP Practices, community settings and home visits.

Key working relationships

Patients, patients families, and carers

GPs, nurses, and other practice staff

PCN clinicians, managers, and colleagues


Core duties and responsibilities:


Working strictly in accordance with specific guidelines and agreed protocols, the Care Coordinator Phlebotomist will assist with the ongoing support of patients, families, and carers in a professional manner - using resources economically and efficiently.


Participate in multidisciplinary and case management meetings, where appropriate, and develop and maintain effective working relationships with health and social care colleagues and other agencies to ensure that patients and those with long term conditions receive a consistent, integrated response to all contacts/referrals.


Phlebotomy
Ensuring that the patient is as comfortable and relaxed as possible and confirming their agreement to the collection of a blood sample

Collection of samples of blood in accordance with training and procedures

Ensuring samples are stored in appropriate containers

Accurate, legible labelling of all samples to confirm patient identity

Checking patient identification against the request and confirming with patient whenever possible

Recording collection of blood sample in patient record in accordance with practice procedure

Ensuring that samples are dispatched to the appropriate laboratories in a timely fashion

Ensuring that anomalies are reported

Notifying the clinician of any significant patient issues

Liaising with and assisting members of the administrative team


Workforce Responsibility
The post holder must remain up to date with mandatory training as required


Environmental Factors
The post holder will be required to drive

The post holder may be required to undertake duties at any location in the community in order to meets service needs

Concentration required for data analysis, tracking patients and meetings, frequent interruptions requiring attention and re-prioritisation of work
