We have an exciting opportunity to join the Grantham CMHT which is part off Lincolnshire Partnership NHS Foundation Trust's Adult Community services.

The Nursing Associate role is new to the NHS and we are looking forward to you joining us in forging the way for Nursing Associates in our workforce.

The role will be part of an integrated health/social care team providing care for service users suffering from serious and enduring mental illness.

Offering nursing interventions in a wide variety of clinical, therapeutic and social forms with the aim of promoting health, wellbeing, self-autonomy and independence.

The CMHT 's are part of the secondary care services, providing assessments and treatment packages for a range of service users aged from 18 years, with complex mental health needs.

The teams use CPA and have a focus on recovery and social inclusion.

The post holders will be required to work effectively as part of the multidisciplinary team in the delivery of high quality mental health care planning and risk management.

This work will involve patients and their carers in the delivery of evidence based interventions/treatments, recovery focused work.

The post holder will have a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person centred care and support within the adult community setting.
