Job Summary


This is a new position which is seen as a critical and evolving post to support and work in St Peters Surgery which is part of the Woolston and Townhill PCN which supports four GP practices.

Delivery of patient centric, integrated care, improving patient outcomes, providing continuity is at the heart of the PCN.

The post holder will work closely with other members of the primary care teams and multidisciplinary team (MDT'S) to ensure the provision of well
- resourced, integrated care closer to home and support patients to manage their own conditions, remain in good health and lead independent lives. This PCN has pockets of deprivation and inequalities and there are significant numbers of care homes within the PCN.

The PCN practices provide acute on the day services and Long-Term Condition (LTC) clinics.

The post holder will support the organisation of the clinics in St Peters surgery for our registered patients and some collaborative PCN clinics, working closely with the clinician teams to ensure patients are signposted, assessed and supported by the most appropriate practitioner.


The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate consistently high standards of professionalism.

The post holder must at all times be aware of the need for confidentiality and integrity.


Key Responsibilities

  • Coordinating clinics, booking appointments, and related admin, collating data and reports and providing feedback to management teams.
  • Manage a database of patients with LTC's to ensure the offer and delivery of holistic wrap around services for patients.
  • Work with practice, operation and reception manager and GP's to ensure appropriate patients are identified from chronic disease registers and offered and supported in accessing additional services.
  • To act as the first point of contact for the Practice and PCN MDT clinic for patients, carers and other professionals making enquiries to the MDT.
  • To support the coordination of proactive MDT care for people living with long term conditions.
  • To work as a key member of the MDT to help support the development of effective MDT meetings.
  • To ensure that actions identified within the MDT are recorded and followed up for completion.
  • Take initiative in the organisation and administration of MDT working to minimise the demands upon the clinical MDT.
  • To ensure the MDT clinicians have access to electronic patient records within the GP practices liaising with partners as appropriate
  • To ensure the MDT clinicians have access to all appropriate clinical supplies and educational resources to support patients and carers
  • The post holder must be aware of and work within available resources.
  • Concentration is required for data analysis, tracking patients and meetings, there may be frequent interruptions requiring attention and reprioritising of work.

Additional Duties
In addition to the above duties you will also be expected to perform the below key activities in line with your job role:

  • Complete mandatory training in line with practice policy and procedures
  • To participate in the staff appraisal process and to undertake for any staff you manage
  • To keep yourself updated on all matters relating to practice policy
  • To provide management supervision where appropriate
  • You will be expected to work collaboratively with PCN practices and other partner organisations, patients, carers, clinicians and other practitioners within the multidisciplinary team in delivering services.
