    Job Summary
    A passionate and skilled Advanced Clinical Practitioner (ACP) can make a meaningful difference in the lives of older adults by providing expert clinical care, focusing on proactive, person-centred support for older adults with complex health needs. This role involves leading comprehensive assessments, care planning, and working collaboratively with multidisciplinary teams to ensure holistic care that promotes wellbeing, prevents hospital admissions, and empowers patients to thrive in their own homes.

    Main Duties of the Job
    The duties of the Advanced Clinical Practitioner (ACP) in the Ageing Well team involve providing high-quality, person-centred care using advanced clinical expertise. The ACP will conduct comprehensive assessments, undertake reactive visits, manage complex health needs, develop care plans, and prevent hospital admissions. The role includes leading on clinical governance, supporting frailty and complex care programs, and working collaboratively with multidisciplinary teams. Responsibilities also cover mentoring, service improvement, and promoting evidence-based practices to ensure the best outcomes for older adults in the community.

    Jurassic Coast Primary Care Network (PCN) is a collaborative group of 3 GP surgeries located in rural West Dorset, including Ammonite Health Partnership, Barton House Medical Practice, and Lyme Bay Medical Practice. Serving a predominantly older population, the PCN focuses on delivering integrated, person-centred healthcare. The PCN's vision emphasizes prevention, supported self-management, education, empowerment, and innovative care models. By working together, member practices aim to provide proactive, personalised, and coordinated health and social care to the local population.

    Job Description
    The postholder provides clinical leadership and strategic support for PCN governance, patient safety, and service monitoring. Key Responsibilities include:
    1. Clinical Practice:
      • Independently assess, diagnose, and manage complex cases, including prescribing and treatment plans.
      • Lead comprehensive assessments for complex care needs.
      • Provide expert clinical advice and support across care settings.
      • Ensure continuity of care through effective communication with multidisciplinary teams.
      • Maintain accurate, confidential patient records.
    2. Leadership & Management:
      • Lead clinical supervision and peer reviews.
      • Act as a role model for clinical leadership and safe practice.
      • Support risk management and quality improvement initiatives.
      • Contribute to incident investigations and service improvements.
    3. Education & Workforce Development:
      • Mentor and support staff development through formal education and training.
      • Participate in recruitment and professional development activities.
      • Promote continuous learning and professional growth within the team.
    4. Research & Development:
      • Implement and promote evidence-based practice.
      • Engage in service audits and quality improvement projects.
      • Disseminate findings through professional presentations and publications.
    5. Policy & Service Development:
      • Contribute to service planning and policy development.
      • Lead and influence the implementation of clinical guidelines and best practices.
      • Foster collaboration across professional boundaries for holistic care delivery.
    6. Professional & Data Management:
      • Ensure compliance with professional standards and IT competencies.
      • Collect and present data for service evaluation and improvement.
    Person Specification
    Qualifications:
    Essential qualifications include:
    • Registered health practitioner with extensive experience in the relevant specialty.
    • Full Master's degree.
    • Registered non-medical prescriber.
    • Learning and Assessing in Practice qualification or equivalent.
