Enhanced Community Support Worker- Neurodiversity - Plymouth, United Kingdom - Livewell Southwest
Description
To work as part of the Neurodiversity pathway to work with children and young people and families post diagnostically on discharge from the Child Development Centre or the Child and Adolescent Mental Health Service.
To support the delivery of evidence-based support in a timely manner The core function of the pathway will be to provide Family, child and young person focused support, Structure group work-themed, drop ins, Child led Play, Sensory support and Sleep support.
They will also support with covering a Telephone advisory line- Morning and afternoon 1.5 hrs to start could be expanded or reduced. To support the delivery of evidencebased parenting programmes for children and young people.
Ensure planned interventions are implemented effectively and evaluated using Routine Outcome Monitoring (ROMS) Maintain standards of care to highest level within available resources.
To maintain confidentiality in respect of the young people at all times. Communicate clearly with children and young people, their families and all other health professional groups. Actively participate in clinical governance and quality improvement initiatives, e.g. clinical audit, essence of care or clinical policy development.
Ensure that children and young people are encouraged to play a part in decision making about the help they need and how they can access specific interventions.
