You will join a team of experienced Personal Advisors and Social Workers who are supported by their supervisors and managers, working closely with a wide range of partners to provide an excellent service for our care experienced young people.


_ Your Role_


As a Leaving Care Worker/Personal Advisor you will support care experienced young people between the ages of 16 and 25 leaving Local Authority Care and assist with their transition into adulthood.

You will hold a caseload of children/young people, with a remit to support pathway planning and transitional plans towards adulthood for 16 and 17 year olds and as the lead professional for 18-25 year olds, leading on pathway planning to achieve success in independence.

You will work closely with colleagues, managers and partners to support our care experienced young people in a wide range of areas of their lives, including sourcing and supporting accommodation, ensuring health needs are being met, and access to learning.


This post carries a requirement to work unsocial hours at times and use of your own vehicle to travel within North East Lincolnshire and outside the county to suit the needs of the young people you will be working with.


You will develop a detailed knowledge of current legislation relating to Looked After Children and Care Leavers and you will be supported to develop your practice to demonstrate the highest social care standards and values.


Within this role you will be working alongside a mixture of young people born in the UK and young people seeking asylum in the UK, with a range of needs and life experiences.


_ About You_


You will be able to work as part of a team, but also manage your own time and diary with the support of your colleagues and managers.

Good quality written and spoken English are essential to this role, as are report writing skills which you will use to write assessment of our young people's needs and prepare plans to ensure they have the right support.
