We are looking for a motivated, creative and passionate person to join our Community and Neurodevelopmental Paediatrics Team (CNP) as a Community Clinic Facilitator.
You will work with a variety of members of the team and have peer support from Ashley who is our other Community Clinic Facilitator across our community services.
Like Ashley, you will be instrumental in ensuring children, young people and their families that attend our community clinic settings, have a positive experience and joining a team whom go consistently go above and beyond their duty of care to improve patient experience and sustain our services.
If you have experience of working with children and their families and are motivated, have an ability to work within a team whilst being flexible, able to prioritise your workload, be innovative and an effective communicator; then we want to hear from you.
Enthusiasm to support our teams to deliver high quality services is a must for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable and approachable.
Core components of this role are:
- To assist the CNP Team by providing a high quality clinic facilitator service within the outpatient setting, ensuring the smooth running of outpatient clinics, meeting the needs of the children and their families and responsible for liaising with appropriate staff.
- To obtain CYP's growth measurements and their observations to aid other's clinical decision making.
- The post holder will work without direct supervision.
- To develop and maintain sound management and organisational skills.
- To act as patient advocate, ensuring privacy, dignity and confidentiality are met.
- To monitor and ensure patient satisfaction and be proactive in managing any issues that are identified
You will have the opportunity to be involved in exciting service transformation plans around further enhancing the environment alongside your core role expectations working alongside the Community Paediatricians and being an active participant within innovative activities and projects that enhance patient service delivery.
We offer specialist assessment, coordination with other services and long-term support to children and young people and their families with developmental conditions or complex health needs.
We work very closely with our therapy colleagues (physiotherapy, occupational therapy, speech & language therapy) and our community and specialist nursing team.
We have a range of expertise areas including assessing and managing children & young people with:
- Neurodisability (neurological problems associated with disability e.g. cerebral palsy)
- Developmental delay or abnormal development in preschool aged children
- Suspected neurodevelopmental disorders, which include:
- Autism Spectrum Disorder
- Attention Deficit Hyperactivity Disorder
- Developmental coordination disorder (dyspraxia)
- Sensory impairment e.g. newly diagnosed visual impairment or significant hearing impairment
- Neonatal presentations with high risk of developmental delay including those with congenital syndromes
- Involvement of the Adoption and Fostering teams performing medical assessments and participation in Adoption Panel Meetings
- Special Educational Needs or Disabilityclinics held at Special Schools and completion of Health advice
We provide a service to children and young people between the ages of years, or up to 19 years if they are attending a special school setting.
