To work as a member of the Urology team and providing patient coordination and care.

The post holder will support ensuring a smooth and efficient service for patients by navigating and signposting them throughout their complex timed cancer pathway.

Navigation will commence from the point of referral for suspicion of cancer and will support the cancer service in implementing and achieving the timed cancer optimal pathway, to achieve definitive diagnosis by day 28.

Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit.

The post-holder will work closely with Cancer Multidisciplinary Services to support and coordinate the care of cancer patients through their complex care pathways, ensuring all patients move through the pathway without unnecessary delays leading to quicker diagnosis and treatment, reducing patient cancellations and DNAs (Did Not Attends), therefore leading to a quicker diagnosis and treatment pathway.

Collaborate and communicate with the CNS and wider Multi-disciplinary Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.
