    The Occupational Therapist Assistant will join Hackney's new Reablement Team, currently based within the Integrated Discharge Service. This team supports residents who have been discharged from hospital and can benefit from reablement, working alongside them as they are discharged from hospital. The aim of the team is to support residents regain their independence and confidence by providing practical support with their care needs, whether they are living in their own homes or staying in the new Reablement Assessment Flats.

    You'll be part of a collaborative service that works closely with families, carers and partner agencies to make sure residents feel supported every step of the way.

    As an Occupational Therapist Assistant you will support Hackney residents to live more independently by carrying out strengths-based assessments, equipment reviews and follow-up visits under the guidance of a qualified OT. You'll recommend and arrange specialist equipment, minor home adaptations and mobility support such as Blue Badges or Tech Enabled Living, ensuring all interventions are safe, effective and meet legislative requirements.

    Working closely with residents, carers and a range of professionals, you will promote wellbeing, identify risks, and provide clear advice or signposting to other services.

    The role includes accurate record-keeping on the council's IT systems, preparing clear reports, and managing your caseload with good time management and professional standards.

    Holding an accredited Trusted Assessor qualification and experience of carrying out occupational therapy assessments and recommending practical solutions that promote independence will be an advantage but not essential. As well as being confident in applying the Care Act and related legislation, you may have a strong working knowledge of specialist equipment, home adaptations, Tech Enabled Living and assistive technology.

    You can make sound clinical decisions, manage your own caseload and record work clearly on case management systems.

    Skilled in communicating complex information with residents, carers and professionals, you build positive relationships across a wide range of organisations. A commitment to strength-based, person-centred practice, continuous professional development and high professional standards is essential.

    An enhanced DBS is required.
