Description

Maintain high standards of patients care, ensuring patients individuality and dignity is maintained at all times, while promoting independence.

Assessing and supporting individuals of any age including children and young people, working age and older adults presenting with mental health concerns.

Completing, accurately documenting and appropriately sharing complex psychosocial assessments, including mental state examination, risk assessment and crisis interventions. Liaising with carers, families and professionals.

Able to actively and creatively engage patients and carers, providing psychological interventions and signposting onward referrals for individuals ensuring holistic care in conjunction with ongoing physical health needs.

Work closely with care coordinators, social workers and all primary health care team members and all others concerned to facilitate safe discharge or transfer of patient care.

Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism.

Use initiative and take responsibility for themselves and the quality of your work and the service you provide to patients, carers and families.

In partnership with patients, carers and other agencies (both NHS and voluntary) create effective, dynamic and robust risk management and crisis plans tailored to individual need, following full pyscho-social assessments.

Offer education and information to patients and carers as appropriate in regards to mental health issues and illness, encouraging empowerment, independence and informed decision making.
