About Us:


The Oaks Care Home is highly regarded in the local area, and has a Good CQC rating, the home provides care for up to 61 older people.


The home is supported by a friendly and committed team, fully trained in providing care for older people who require residential care, nursing care or who are living with dementia and other complex conditions.

We are seeking a Care Practitioner to join our friendly team, at The Oaks Care Home, in Lexden.


What does the job role involve?

  • To only undertake delegated duties, which are within, assessed competencies for the Care Practitioner Framework
  • To undertake clinical tasks such as physical observations, urinalysis, skin inspection, aseptic technique including simple wound dressings, blood glucose monitoring, gastrostomy care and administration of gastrostomy tube feeding
  • Oversee nutrition and hydration management
  • Undertaking designated aspects of medicine management in line with legal and company policy
  • Assisting nurses with planning and delivering end of life care, alleviating pain through both pharmacological and non pharmacological means
  • Ensuring safe medication orders, receipts, recording, storing and disposing of medication in line with legal and company policy
  • To provide support to the Team Leader as required.
  • To support the Team Leader in preparing and implementing procedures as directed.
  • To provide the agreed level of support to residents as defined in their care plan.
  • To supervise a group of care assistants as agreed with the Team Leader.
  • To act as mentor to new care assistants as delegated by the Team Leader.
  • To assist in the planning and monitoring of resident care.
  • To assist in the implementation of residents planned care.
  • All other duties involved in this role.
  • Must have a NVQ3 or be actively working towards this qualification
  • Ideally experience as a Care Practitioner would be an advantage
  • Must be passionate and willingness to learn as training will be provided
  • Able to work well as part of a team and independently
  • Able to follow and prioritise tasks and responsibilities set
  • To have the values of making a positive impact
