Forest Homecare is a well-established family-run care agency that operates throughout Essex & Suffolk. We have an exciting opportunity available for a Care Coordinator on a full-time basis.

Working from our Head Office in Sible Hedingham, the successful Care Coordinator will have the responsibility of high-quality domiciliary care services are delivered to service users through efficient management within the contracted service area.


Your main duties will include:


  • To ensure that high quality domiciliary care services are delivered to service users through efficient management within the contracted service area
  • Witham and surrounding villages.
  • To manage on a day to day basis the coordination and preparation of rotas, care packages and care staff.
  • Coordinating involves not only the day to day management of care packages and the maintenance of rounds on Care Planner, but also liaising with the FHC Review Officer to ensure service user and carer concerns are appropriately and effectively addressed and fully resolved, referring to the Office Manager when necessary and as required.
  • To address developmental needs of care staff in the team and performance levels through supervision (one to ones) and to ensure observations of practice and other compliance tasks are completed according to the target dates.
  • You will arrange and participate in group supervisions/team meetings.
  • To support and monitor new care staff during their three month probation periods and to ensure all required documentation and observations are completed and new care staff receive carefully selected rounds to support them to complete the Care Certificate and enhance staff retention.
  • To participate in the oncall/out of hour's duty rota and maintain up to date oncall documentation at all times.
  • To contribute towards the development of domiciliary care services in Essex to meet the changing needs of service users and commercial contracts with Essex County Council and local health authorities.

Area Coordinating:


  • Supervise and support the care team in day to day service delivery to the highest possible standard, ensuring all visits are covered and that no visits are missed or cancelled
  • To reallocate visits and rounds speedily effectively and safely at all times, ensuring care staff are spoken to directly and instructions confirmed by text to avoid any misunderstandings or missed visits
  • To ensure oncall staff are telephoned or seen daily to be updated, e.g. hospital admissions, concerns about service users and general issues
  • To ensure all changes that have taken place out of office hours are entered onto Care Planner the very next working morning
  • To alert the FHC Review officer regarding any possible visit reductions/increases/changes required to a care/support plan or when risk assessments need to be reviewed
  • To maintain a working knowledge of relevant UK/EC legislation, CQC Essential Standards for domiciliary care, best practice and codes of practice that impact positively on the provision of community based care services

CQC minimum standards and ECC Contract Compliance

  • To ensure the Forest Homecare annual compliance/supervision programmed which meets the Care Quality Commission's Compliance requirements is fully met and within the required time frames for all care staff.
  • To deliver appropriate support and induction to new care staff during their entire probationary period and ensuring they are signed off at the end of 3 months unless a formal request to extend the probationary period has been authorized by the Office Manager
  • To ensure new care staff are booked in for their induction rounds the week after completing induction training and that they are allocated to rounds with Senior Care Assistants or experienced and high performing care assistants only

Service delivery:


  • Managing the day to day visit changes, ensuring service is maintained at all times to all service users, you may be required to perform rounds when capacity is challenged and work cannot be covered by existing resources
  • Dealing with service users' queries and responding to them effectively and speedily
  • Ensuring whenever possible service users or appropriate others are notified in advance of any operational issues affecting their service delivery including when we are running more than 30 minutes late
  • To ensure compliance with the company's Safeguarding of Adults procedures, alerting the Office Manager or Head of Administration immediately if there are any potential safeguarding issues. To complete, as required, safeguarding alerts for approval by the Office Manager.
