Street Outreach City is a rough sleeping service. Our service users have multiple and complex needs including substance misuse, offending history and Mental/Physical Health.

Our aim is to work with individuals to support them into suitable accommodation.

The main function of the role will be:

To assess individual service user's skills and needs and to work positively with risk
To support service users to engage with a process of resettlement into appropriate longterm accommodation
To work closely alongside other partnership agencies
