We are looking for a WAM Community Nursing Team Triage Administrator to join ourCQC rated 'Outstanding' Trust.

As a CommunityNursing Team Triage Administrator,youwill support the Community Nursing service deliver care to people in their own homes byassistingin the triage of new referrals as well asprovidingadministrative support to the team.

  • To be the central point of contact alongside a trained clinician for all new referrals into the District nursing service, taking messages, obtaining information for new referrals, advising patients about visits, referring as appropriate and answering routine enquiries.
  • You will assist in the triage and allocation of new referrals, advise patients about visits, refer as appropriate and answer routine enquiries.
  • Type confidential correspondence, reports and other documentation to a high standard within agreed timescales.

The "must haves" for this role:

  • Excellent interpersonal and communication skills
  • Good IT skills, with the ability to navigate around various systems
  • Work effectively in a team and independently, using initiative
