Familiarity with IT systems About Us Primary Care Network 4 (PCN4) is a network of Bradford City practices working collaboratively across the network area to look after the health of our local population.

The PCN which comprises of 5 local practices with a total population of around 42,000 registered patients.

Main Duties of the Job You will work closely with the Primary Care professionals within the PCN to identify and manage a caseload of identified patients.

The role will include coordination of clinics, inputting of clinical data and direct patient contact to encourage uptake of services.


The Project Coordinator will support the coordination and delivery of MDTs within PCNs; ensure close working relationships with other PCN roles Support Quality and Outcome Frameworks and Local and National Targets.

Job Description Job responsibilities Work with Primary Care professionals within the PCN to identify and manage a caseload of patients, be involved in co-ordination of care and where required and as appropriate, refer people back to other health professionals within the PCN Use data to proactively identify and work with a cohort of patients to deliver personalised care Help people to manage their needs, answering their queries and supporting them to make appointments Ensure that people have good quality information to help them make choices Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing Support the coordination and delivery of MDTs within PCNs Provide support in setting up clinics Support Quality and Outcome Frameworks and Local and National Targets, Contribute to effective information management within the team including collation, analysis and reporting of data Skills and Competencies Knowledge and experienced in using SystmOne Knowledge of the different Health Checks available Familiar with different methods of data collection Excellent planning and communication skills Active and empathic listening Effective questioning Building trust and rapport Shared agenda setting On-going development Buddying with peers Qualifications Essential Meet DBS standards Willingness to work flexible hours Ability to use Microsoft office and word Understanding of safeguarding policies for children and vulnerable adults Ability to recognise and work within limits of competence and seek advice when needed Experience of working in health and social care and other support roles in direct contact with people High level of written and verbal communications skills Knowledge of and ability to work to policies and procedures Desirable Ability to travel across locality on regular basis Own transport Experience of working with elderly or vulnerable people
