Description

Job Title Personalised Care Lead - Mid Cornwall - St Austell and Surronds

This is to cover St Austell and surrounding areas including (Roche, Bugle, St Dennis, Indian Queens, Fraddon, St Stephen, Duporth, Pentewan, Mevagissey, down to Portscatho/Veryan.

We want to recruit from this area to minimise our carbon footprint.


Salary £24,500 annual salary

Mileage paid at 45p per mile


Reports to Locality Lead


Place of work Community based.


Working hours 37 hours per week

8:30am-4:30pm Monday-Thursday and 8:30-4pm Friday _- Flexible by half an hour each way. (i.e., 8-4pm/9-5pm)_

Your Role in a Nutshell


This role involves working in a small, flexible team that tackles a wide range of tasks to meet client needs.

It's demanding, working with vulnerable individuals in complex situations.


Your main responsibilities include:

  • Conducting holistic assessments for clients in your area, looking at all aspects of a person's life.
  • Visiting up to 4 clients daily in their homes.
  • Providing compassionate, individualised support that covers unique needs.
  • Using a holistic approach to address immediate care needs and root causes of health challenges.
  • Working urgently with individuals at risk of admission to prevent it.
  • Utilising reablement strategies to empower independence through innovative approaches.
  • Collaborating with diverse professionals and maintaining accurate records.
  • Adjusting support plans based on evolving needs and advocating for individuals' rights and preferences.
  • Staying updated on industry trends and contributing to service improvement.
  • Participating in team meetings, case conferences, and professional development.
  • Building strong relationships with individuals, families, and caregivers.
  • Collaborating with Support Brokers to implement sustainable and personalised solutions.

Qualifications and Attributes

Essential Criteria:


  • Eligibility to work in the UK.
  • Access to own vehicle with business insurance and a confident driver.
  • Strong communication, troubleshooting, and time management skills.
  • Proficiency in using computers, including case management systems and tablets.
  • Multitasker that can handle a variety of tasks to make goals achievable.

Desirable Criteria:


  • Coaching & Mentoring qualifications.
  • Previous experience overlapping with that of Humans Cornwall.
  • Experience with care assessments and risk assessments.
  • Valid moving and handling certificate.
  • Familiarity with client case management systems.
  • A good understanding of your local community.
