Description

CERT was set up in 2014 to provide intensive, bespoke packages of care to service users in their own homes, as an alternative to being in Locked Rehabilitation, out of city hospital placements.

The team works with up to 45 service users with mental health problems, personality disorders and/or learning disabilities to support them to move from hospital into their own tenancies.

Referrals are primarily received from inpatient acute and rehabilitation services.

Service users are supported by a full multi-disciplinary team with the aim of enabling them to live independent and fulfilled lives.

The team provides intensive quality interventions between 8am and 8pm, 7 days a week.

As a Care Co-ordinator you will be responsible for managing a small, but intensive caseload service users and supervising a mini team of recovery workers.

You will also be expected to provide leadership and coordination to the team on a day to day basis.
