We are recruiting fora Trusted Assessor Assistantwho wishes to join #TeamIOWNHS.

The post will focus primarily on working alongside the Trusted Assessors within the Integrated Discharge Team (IDT) in a patient facing role.

You will work collaboratively with the wider IDT and work within a Multi-Disciplinary Team to help identify a safe and appropriate discharge for patients in the acute hospital wards.

To assist with reviewing patients on the acute wards, working closely with the Trusted Assessors and the wider IDT to ensure all information we have about patients is complete and up to date.

Review patients and their notes on the wards to make sure they are at the same level as when assessed.

Work with line manager if further information or communication with patients is needed and work with MDT with information gathering about specific patient.

This year Isle of Wight NHS Trust and Portsmouth Hospitals University NHS Trust have strengthened our partnership and formed a group giving us an opportunity to work together to be innovative in improving patient care and outcomes as well as improving the experience and opportunities for our people across the two organisations.

We have also been working with other local NHS Trusts across our region to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight.

Subject to approvals, the aim is for the new Trust - Hampshire and Isle of Wight Healthcare NHS Foundation Trust - to be formed by April 2024.

People employed in our community, mental health and learning disability services by the Isle of Wight NHS Trust on 31 March 2024, will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) on 1 April 2024.

The Isle of Wight is a unique and stunning gem located off the South Coast of England, a short ferry journey from Portsmouth and Southampton, with journey times taking from as little as 10 minutes.
