Description

A duty to advise senior nurses of potential problems or errors within the range of assigned tasks Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment There will be a requirement to undertake home visits Any other delegated duties appropriate to the post Ability to assess and manage the range of conditions encountered in general practice using a variety of assessment tools and consultation models appropriate to the patient and situation using physical and clinical examination skills to inform the assessment and decision-making for the on-going management of the patient.

Advanced communication skills that include skills of influencing and negotiation to enable information to be delivered in understandable formats for patients and behaviour change supported where necessary.

Effective multidisciplinary and multi-agency team working, alongside the ability to work independently and accept professional accountability and responsibility for the delivery of whole episodes of care, and supporting and developing others in the general practice nursing team to collaborate effectively, ensuring nursing care is guided by precedent and clearly defined policies, procedures and protocols.

Delivery and co-ordination of evidence-informed, person-centred and negotiated care across the age spectrum.


Use of technology to support independence and patient self-care at home to improve self-care and reduce exacerbations Role modelling and embodiment of non-judgmental, value-based care encompassing the 6Cs in practice and expectation and promotion of these values in other team members.

Competence in delivering evidence-informed care across a wide range of minor acute and LTCs, ensuring effective evaluation of therapeutic and other approaches to condition management alongside ability to assess patient concordance.

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.


Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care with referral to GPs as necessary. Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment.

Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used.

Perform any investigatory procedures requested by the GPs.

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC) Participate in the administrative and professional responsibilities of the practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Contribute to the assessment of service needs.

Assist patients to identify their health needs. Contribute a nursing perspective to the practice development plan. Manage and organise individual patient consultations. Aware of identification and reporting procedures related to professional standards.

To attend staff meetings. Ensure effective relationships and communications with the PHCT and other agencies. Support the Practice clinical governance agenda. Identify changes to clinical practice that are required to implement evidence-based guidelines.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports. Attend and participate in practice meetings as required. Assist in the formulation of practice philosophy, strategy and policy and develop appropriate protocols.

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. Contribution to the development of local guidelines and policy locally and regionally, and nationally where appropriate.
