We are looking for a hard-working, enthusiastic, highly motivated and experienced or developing Advanced Clinical Practitioner from a nursing or AHP (with the ability to undertake or have completed Non-medical prescribing) to join our Frailty Team to support delivery of innovative new ways of working across Medicine and Emergency Care, particularly to support the frailty service in SDEC and establishment of the Frailty Virtual Ward.
Annex 21 of Agenda for Change Band 8a applies to this post.You will already have or are willing to work towards an advanced practice qualification at master's level.
The primary focus of the role is to work as an advanced, autonomous practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis, treatment and discharge of patients.

Main:
duties of the job
- Working closely with multidisciplinary team to ensure seamless, evidenced based patient care pathways.
- To clinically support other Clinical Practitioners across Medicine and Emergency Care in the service.
- To act as a known source of clinical expertise with your specialty and provide formal and informal support and training for all grades of clinical staff.
- To lead in the development of evidence based clinical guidelines / protocols within your specialist field.
- To undertake the triage of referrals using advanced clinical reasoning and expert knowledge, directing referrals to the most appropriate service / clinician.
- To request radiological and haematological investigations & independently Interpret / analyse results working towards the normal scope of ACP to form an accurate diagnosis, and subsequently select the most appropriate patient management.
- To manage a defined specialist caseload, using evidence based / client centred principles for patients with highly complex and challenging specialty conditions.
- To contribute and work effectively and efficiently as a member of the team to deliver holistic high quality patient care.
